Ok I was wrong
it would not work because I had an older version installed previosly.
I managed to resolve this by
1. complete removal of old version ( all hidden folders / files ) use revo or similiar to clean completely
2. IMPORTANT remove previous firewall entries created by Medicines before reapply with this version.
After these steps this worked perfectly

it would not work because I had an older version installed previosly.

If you are not familiar with advanced settings in windows firewall use the program firewall app blocker from sordum its a free app which can help you delete the DFoXT_iCareFone_-_iOS_Backup_Tool_TNS entry.
